    Evidently, ADHD meds increase the risk of certain types of glaucoma by nearly 150%. The numbers might be small but the risk is real. If you are prone to glaucoma, as diabetics often are, you might want to think twice about taking ADHD meds. You do yourself an injustice by minimizing these results.

    I just recently started to take medication. I am on Medikinet. The first difference I am noticing is that it in general improved my drive. And a lot of the tiredness I feel is reduced. What I still struggle with is to guide my attention. So I see myself driving at 200 mph per hour now in the wrong direction at times. That's why I was looking into ways to better structure my day and be more mindful of the activities I want to do, so I do not run into traps that drain my attention. Another thing I did notice is that I am feeling emotions that I am not use to. I panic, or get stressed earlier when some appointment in the future is coming towards me. Before I could see the event and just don't feel anything. Until it's immediate. So there is an improvement. I also feel now some other emotions here and there that I am not used to. For example memories of my past when they come up I have a stronger emotional response to. But yeah long story short. I need to work out a system that breaks down my day, and keeps reminding me of where I am at right now and what next needs to be done. I am gonna figure something out. Thanks for the video.

    I really like how you broke down the actual _real world numbers_ when discussing ADHD medication being correlated with glaucoma. You see sensationalism in the news all the time based on health issues like this where a medication/activity/food significantly (in the literal sense of the word) raises the risk of some potentially serious negative outcome from practically zero to... still practically zero. That's not to say we shouldn't appreciate risks, but we should understand them and treat them proportionally. If ADHD meds made people 10x more likely to be struck by lightning, they'd still be a good choice. It's worth considering something like the 2.5x increase when someone has multiple overlapping risk factors, though. Take, for example, a 55 year old diabetic Asian woman with high blood pressure: She already has five factors increasing her risk. Is adding a potential sixth one worth it? Maybe, but maybe that's a case where a combination of other treatment modalities would be more appropriate.

    Hello Dr Berkley, is there any research on how GLP-1 medication such as Mounjaro and wegovy may impaire the effecacy of stimulants since these medicines act on the dopamine receptors and reduce food cravings.

    I got diagnosed with ADHD AFTER being diagnosed with Type 2 diabetes. I had no idea my uncontrollable sugar cravings had a cause I thought I was just irresponsible and depressed lol. Learning about dopamine seeking particularly through food was eye opening.

    I find listening to these videos, ie a topic of great interest, combined with doing a perfunctory task go well together. It seems like the automatic task is undertaken easier and better by my brain being engaged in the listening and I listen better because I'm undertaking a mundane task, maybe its just the movement that helps. So right now im listening and sorting laundry and doing very well at both.

    After surviving what I refer to as having been a train wreck of a life for 53 years, it was such a relief to finally have an answer. Learning to understand how much my ADHD has played a major role in the things that I have done, and being able to forgive myself for the things that were out of my control especially prior to being diagnosed, has made me determined to live my remaining year's much better than I could have hoped had it not have been for getting an answer. I thank you for your excellent resources that I use to navigate life working with rather against my ADHD symptom's. I do however have another medical disorder that along with ADHD makes life extremely challenging, and this too was only diagnosed late in life when I was 49. The condition is narcolepsy (NT1) with cataplexy, and I am wondering if you have come across any credible research regarding living with both, and whether there is any useful information that you would be able to share in a future video for people like me? Since receiving both diagnoses' I have found that there are quite a lot of people who have both, so even if there currently isn't a lot of information regarding this combination perhaps this is something that either you or perhaps another professional that you know in this field might consider exploring. Currently I use my knowledge of both separately but it would really help to know how to manage and treat both disorders together. Thank you again for being an excellent resource for everyone who lives with ADHD.

    Can confirm everyone's experiences here as an adult diagnosed in my 40's. So much anger and disappointment, but also relief that my experiences and pain finally had a reason behind them. I would like to add my experience of 're-grief' to the conversation. Re-grief occured when I finally accepted (waded through denial) that I needed supports in my workplace. Through an employment support program, I was given access to monthly sessions with an ADHD / Executive Function coach. The grief lasted several months as I was so disappointed that I couldn't "just pull myself together". Then there was shame that I needed this support. As time goes on and we continue to work together, I feel a bit better. Other days I just wish for retirement so I don't have to feel like an inferior employee every single day. It is not easy.

    So it seems yet again that ADD vs ADHD; changed to just ADHD to be inclusive which for me is not productive in having others understand that my mind works a certain way. Sorry but it feels we have just convoluted the two into one big ball of problems people want to easily dismiss all of it because there is no sympathy let alone wanting to bother to understand that there are specifics, all of that is thrown out the window by people with the simple phrase everyone is different and or well I do that too!

    This made me feel so validated. As a kid, my ADHD was more manageable. But at 29, I’ve realized how debilitating it is. I explained this to my therapist yesterday, actually. 30 minutes feels like 2 minutes. It’s not about caring, but about not having that structure of time.

      Totally agree with you. I absolutely care. For me it's like there is an intellectual part of my brain that knows what tasks can reasonably get done in 10mins, but when I'm doing something that intellectual construct doesn't help me at all. That part of my brain has absolutely no application or meaning for me in the process of doing the tasks. 2mins, 20mins or 2hrs it all feels like "about ten minutes" to me in the doing of stuff.

    Thank you for your videos. I definitely recall having delayed internal speech and going through the progress toward internal speech at a much later time as mentioned in the video, but if I’m like a number of other adhd people here and I believe that I am as a bunch as saying the same thing is that now their internal speech is _over_ active and doesn’t stop and in fact is speaking so fast internally and it is nonstop that there’s some sort of inhibition that never got turned on and therefore over takes all other sensory processes within the moments of trying to listen externally. . Ie. If I look at someone, I can’t hear them, if I don’t look at them then I can listen and understand them better, if I use a fidget ring on my left finger with my left thumb, it engages the right hemisphere which then inhibits the left hemisphere and therefore quiets and inhibits the language production areas so I can then quiet the internal speech and then be able to focus on the external conversation. Whereas if I put the ring in my right first finger and spin it with my right thumb, it doesn’t stop the internal speech at all and if anything activates it more. We need to be looking at the inhibitory connection between the parietal lobes and frontal lobes, ipsilateral as well as contralateral connections. And in addition, we need to be looking more at the involvement of the insula with not only language processing but also emotional regulation with fear, anger and sadness, which also plays a huge part in adhd. There are a few dozen mri and DTI studies that can be done on these topics. Again, thank you for your insightful videos.

    My problem not formally recognizing RSD is that my brain was not able to equate difficulty self regulating with rejection sensitive dysphoria. To me they are 2 different things and so I struggled to identify with My inattentive ADD until I learned about RSD. While the approach to dealing with them might be the same, experience of RSD is not the same, and not having it listed as it's own symptom, might be difference between someone recognizing themselves when looking into the possibility of having ADHD / ADD and not.

    Interesting video Dr Barkley. I have to say, I'm not quite sure on this one. Most of the people I know have worked with that have ADHD describe almost a hyperactive inner voice or a racing mind of inner speech. The idea that it could be from a lack of inner speech doesnt mesh too well with my experiences. Although, it could relate to difficulties filtering and organizing inner speech for task relevant features perhaps? Curious to hear what your take is on this.

    Dr. Barkley- why hasn’t CDS been included in the DSM yet? Do you think it will eventually?

      There was not enough evidence on the validity of CDS as a separate disorder back in the years 2008-2012 when the DSM was being revised. That has all changed since then so let’s hope it gets into the next one, DSM6 whenever that gets created.

    Two of my seven children were behaviorally reactive to certain food dyes. This was not a subtle thing; it was as if someone toggled a switch and the unhinged zombies came out. (I remember once coming home to a child in the center of the living room, neck cranked up, staring at the ceiling fan, as he spun to keep pace. His dad had given him colorful sugar cereal.) We rarely experienced this as I did the shopping and never brought things into the house that any of the kids were allergic, sensitive, reactive to it intolerant of. Not what I’m wondering is, as so many sugar foods marketed to kids are loaded with food dyes, if people are attributing to sugar what’s actually sensitivity and reactivity to some food dyes.

    I was initially pleased after being diagnosed at 33 as I finally had an answer as to why I found everything more difficult than it appeared my peers did, but a year on I actually feel more poignant about it. Looking back at all the things I did as a young child, struggled to maintain friendships, never clicked into place within groups, risky behaviour as a teen, needless car accidents, and to me now looking back it seems so obvious. Even early years of motherhood, I was in a hole, and no one spotted it or saw it, I just had a messy house and it seemed like I couldn’t keep on top of things. Without the boost of awareness on social media I wouldn’t have figured it out about myself. I will be looking into some counselling to try and move past it all. I also find that people can be very nonchalant about it even after learning I have it, which I think is due to so many creators making videos listing things that most of the population also find difficult, and not stressing the major difference is we do these things all the time, and without any ability to stop. So many comments about oh well everyone’s a little adhd 🤯

    I still to this day (44yrs) have trouble working things out fully in my thoughts. I definitely work through a thought process better by saying it out loud. It's only since being medicated that my external thinking is more fluid. Before this I had many long pauses and much more trouble recalling my words. But, because of my external thinking, while verbally working things out I say the wrong conclusion and also correct myself a few times. This to others around me invites interruptions to correct me, which in turn stops my process while someone else worked out the conclusion. This frustrates me so much.

    Hello Dr. Barkley. I know this video is a bit old but I wanted to ask you something that may be important. I've seen a lot of comments through the internet saying Guanfacine may have long-term effects on the way our pre-frontal love is wired. I havefound this study from 2021, would it be possible for you to adress this question here or in a video if it has any evidence for such feats ? The possibility that it can mildly "cure" adhd is probably part of why this information has been shared so many times, but as a profane individual in the field I cannot say if that's actually what the state of the art actually says about this medication.     After reading the comment section... its clear most dont seem to understand G+E=P. Genetics + Environment = Phenotype. Phenotype is the outward expression of one's genes. We know the phenotype for ADHD can manifest differently, but how much is one's environment influencing ADHD really? I see/hear people in here and in other chats etc as well as everyone I have told about my adhd say things that either place it all on genetics or on environment. As a breeder and student of genetics that equation has taught me a lot. For example there is a 20% average (varies by breed) of genetic predisposition to hip dysplasia. BUT dogs can get hip dysplasia without those genes. On top of this, dogs with the genes for HD, dont always have symptoms of HD. And to finish this off... dogs with the genes for HD + environmental aspects like jumping off high places/climbing stairs too much/accident to their back end, will INCREASE the likelihood of HD symptoms manifesting. We know that ADHD can occur from injuries. We know that people can have ADHD symptoms without actually having ADHD genetically. We also know people with ADHD can live highly successful lives with no knowledge they even have ADHD etc etc etc. It clearly is possible to reduce or exacerbate one's genetic ADHD through environmental cues. We have research that even the Doc has presented, to back this idea that we can influence our ADHD. If ADHD is not caused by sugar intake, is there any positive or negative correlations with carbs that the Doc thinks is valid?? I guess I dont blame all the people who think ADHD can be caused by just environment. Most general practitioners only have cursory knowledge unless they have learned more of the details. And most dont know ADHD is a genetic thing because it is very sneaky and can hide itself. haha
